Supplemental Insurance Programs

Frenchman ’ s Creek offers supplemental insurance programs to employees who are eligible to participate in the Benefits Program. We presently offer vision and voluntary term life insurance for yourself, your spouse, and children. We also offer Group Accident, Group Critical Illness and Group Indemnity Medical as well as an Identity Theft Protection Program. Please see Human Resources for plan summaries. Limitations on life insurance may apply based on age. Please refer to Summary Plan Documents provided by the life insurance carrier. Frenchman ’ s Creek offers two Flexible Spending Accounts for all year-round, full-time employees. The Medical Care Flexible spending account allows you to set aside on a pre-tax basis to pay expenses not currently reimbursed by your group health plan, such as co-payments, deductibles, coinsurance, dental services, vision care, etc. The Dependent Care Flexible Spending Account allows you to set money aside on a pre-tax basis to reimburse for the cost of dependent care expenses for children under the age of 13, or for the care of an incapacitated spouse or other dependent when whose services are required to allow you (and your spouse) to work. Maximum amounts are set forth by law which limits the amount of money that can be set aside in these accounts on a pre-tax basis. Careful calculation of the amounts you want to redirect to each spending account is very important. Any money remaining in your account at the end of the Plan Year is forfeited. This rule is an Internal Revenue Service “ Use It or Lose It ” regulation. For information regarding Flexible Spending Accounts, please contact the Human Resources Department. Upon termination or other qualifying event, health, dental and vision insurance will continue through the end of the month in which an employee ’ s employment ends. Under the provisions of the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA), employees and their dependents covered under the group health, dental and/or vision plan(s) of Frenchman ’ s Creek have the option of converting their medical, dental and vision insurance coverage to non-group coverage for up to 18 months at their own expense. Details on COBRA coverage will be provided to employees at the time of termination or other qualifying event. Continuation of Benefits All Frenchman ’ s Creek employees are covered under Frenchman ’ s Creek ’ s Workers Compensation insurance. This insurance provides compensation for certain lost time, medical expenses, loss of life or disability from injury or illness arising out of or in the course of your employment with Frenchman ’ s Creek. To protect yourself in case of injury or illness which is considered work-related (no matter how minor the accident may appear): You must report your injury or illness immediately to your supervisor, Security and the Human Resources Department.
